Vehicle Accident Guide
If you are involved in an accident, call 911 right away. Then, contact the Delivery Lead as soon as you can. Once you have spoken with emergency services and the Delivery Lead, follow additional instructions listed below.
๐ธ Get a photo of the other driver's insurance and license.
๐ Show the other driver CFA's liability insurance.
๐ Take photos of the damage; get several angles of each vehicle.
๐ชจ If the damage involves a non-vehicle object, take pictures of that too.
โ If you use a CFA vehicle, don't drive it until the Delivery Lead authorizes you.
๐ If the vehicle cannot be moved, call Lubbock Wrecker and have it towed to the off-site office (6313 Homestead Ave) on the south side of the building. DO NOT have it towed to their storage yard.
๐ฎ If there is no urgent medical emergency, please complete the Driver's Crash Report, otherwise known as the "CR-2" or "Blue" form below. If the police arrive at the accident, please ask them for the police report instead.
โ Complete the Vehicle Accident Form and send it to the Delivery Lead before the end of your shift.
Lubbock Police Department
In an effort to reduce the number of calls for service and respond to higher priority calls more quickly, the Lubbock Police Department has adjusted its call response to certain traffic crashes. If you are involved in a minor traffic crash with no injuries, involved parties will exchange and complete a CR-2, or Blue form, in lieu of waiting for an officer to respond. A CR-2, or Blue Form, is a Driverโs Crash Report completed by persons involved in the traffic crash.
Please use the Driver's Crash Report for non-emergency crashes where a police officer was not able to come to the scene.
